Forms
Bill of Lading
A Bill of Lading (BOL) is a legal document issued by a carrier (such as Q-Trans) to acknowledge the receipt of goods for shipment. This is to be given to the driver upon pickup. It also serves as evidence of ownership and is often required for the release of goods at the destination.
Credit card authorization
The credit card pre-authorization form is used to process a one-time shipment by obtaining approval from the card issuer to charge the stated amount to the customer’s card. This can also be done over the phone. Once authorization is confirmed, we can proceed with the shipping service.
Customer registration
